When you invest in a beautiful piece of art, the last thing you want is a delay at the border. For collectors in the United States, certain shipments require an extra step to ensure your artwork arrives safely and on time. US Customs and Border Protection (CBP) uses Form 5106 to identify and clear high-value international shipments.
Completing this form is quick and straightforward, and it helps keep your new acquisition moving toward your wall without interruption.
Why do I need to complete this form?
This is a federal requirement for customs clearance. The form collects personal identification details that only you, as the recipient, can provide. For privacy and security reasons, neither FedEx nor Hypergallery can complete this on your behalf.
When and how will I receive the form?
FedEx should contact you after your shipping label is created or sometimes while your parcel is in transit.
You’ll receive two emails at the address linked to your account:
- Notification email

DocuSign email with the CBP Form 5106 (sent from
dse_NA3@docusign.net)
Tip: Check your inbox and spam/junk folders. If you signed in using Apple ID, check your iCloud mailbox.
Can I submit the form proactively?
Yes! Once you have your FedEx tracking number (provided after dispatch), you can:

- Save, and email it to: paperwork@fedex.com
Important:
- Subject line: ONLY your FedEx tracking number
- Leave the email body blank—no text or extra info
What information will I need?

Identification number:
- US citizens, Green Card holders, or visa holders: Social Security Number (SSN) or Employer Identification Number (EIN)
- Non-US residents: If you don’t have an SSN, request a CBP number via the form
You can ask us for a copy of the the FedEx CBP Form 5106 if we haven't already provided one to you.
Is there a deadline?
Yes! Complete the form within 5 business days of receiving FedEx’s email.
If not, your package may be returned to sender and relisted under your account.
What happens after submission?
Customs will process your information and release your shipment. This can take up to 72 hours.
If your tracking shows no movement after that, contact us so we can investigate.
Once cleared, FedEx will deliver your package. If more details are needed, FedEx will reach out directly.
